2009 Low Income Home Energy Assistance Program (LIHEAP) Cooling Program :

Muslim women Resource Center is a contracted agency with CEDA to assist qualified low income families in Chicago and vicinity area apply for the Low Income Energy Assistance Program. In order to better coordinate the application process, families with:

• Elderly (aged 60 or over)
• Disabled members
• Children less than 36 months old or
• Members with medical needs (need Proof of medical Certificate) can come to register.

Registration will start July 6th, 2009 at Muslim Women Resource Center

All applicants should bring along their original copy of the following documents at the registration:

• Applicant and all family members’ Social Security Cards (for proof of SS#)
• Applicant and all family members’passport, birth certificates, driver’s licenses. State I.D. cards If you are not a U.S citizen, you must bring your alien cards (for proof of date of birth)
• Proof of family’s gross income for the last 30 days from all sources of all family members (e.g. check stubs, pension statements, IDHS Grant Information Request Form, copy of Social Security or SSI Checks, award letters from the government, etc.
• Copy of Medicaid card from IDHS (for proof of IDHS case number)
• Current electric and/or gas billing statement.
• Rent receipt in the last 30 days.
